Care ConnectionSo much goes on when caring for a seriously ill loved one. With doctors, nurses, relatives, neighbors and friends all looking for information, good communication with all these people can become overwhelming. That is where a Care Connection can help. A Care Connection is designed to help you balance the many demands of providing care for an ill friend or relative. It lets you use technology to keep everyone in the family updated at the same time and at one location. It gives you space to ask for help and allows family and friends to contact you.

Getting Started
Enter a Care Connection Code, provided by our staff, and you will have access to a personal caregiver webpage. Login whenever you want to post new information about your loved one and the status of his or her care.
Use email to tell as many (or as few) people as you want about your Care Connection. After that, they may check in as often as they want to find out what is going on with the patient, the family and what you may need. Anyone who checks into the website can even leave a message of encouragement if they wish.
